Required Forms

In order to be an eligble member and player, you must fill out the following required regisration forms and turn in a copy of these forms to Connor Baker, the U of I Rugby Team Secretary, at practice or after contacting him.

1.) Sports Club Informed Consent (PDF)

• You must download, print, and fill out this form.  Make two copies, one for yourself and one copy MUST be turned in to the Team Secretary.

2.) Campus Recreation Emergency Card (online form)

• Click on the link and fill out the form.  Before submitting, make sure to print out a copy for yourself and another copy to turn in to the Team Secretary.

3.) CIPP - USA Rugby Membership Registration (online and PDF form)

• In order to be eligible to play and to be covered by USA rugby insurance for injuries sustained during a game, you must follow the link register and pay for your YEARLY USA Rugby membership.  Follow the onsite instructions and print out a copy of the forms to be turned into the Team Secretary.

If you do not have these forms filled out, submitted, and paid for, YOU ARE NOT ELIGBLE TO PLAY!
